Citing a “litany of failures,” the National Transportation Safety Board today formally placed the blame for last September’s deadly San Bruno explosion and fire squarely on PG&E. Investigators concluded that the weld in the pipe that ruptured was defective when it was installed in 1956 and that PG&E would have known had anyone bothered to check. Chronicle has more.
